Tambrands, the former maker of Tampax, generously gave this memo and its attachments to this museum.

Below: The January 1964 ad in 'Teen magazine is still attached to the Tampax memo. I cyber-chopped off the right 2/3 of the page to save space. The ad side of the page (to the vertical line) measures about 3 x 11" (7.6 x 27.9 cm). See more white in menstrual ads - here, too.
The stain and punched holes came with the Tampax donation.
